Blue Cross nationwide PPO plans are now available in Arizona

These Blue Cross plans give you access to every Blue Cross Blue Shield provider in America, and are ideal for families with children going to schools outside of Arizona.

These high quality Blue Cross plans are not ObamaCare ACA plans so they can decline to give you a policy if you have certain medical conditions. All annual ObamaCare preventive services are included with no cost to you. These include a colonoscopy (one every five years), immunizations, screenings for depression, diabetes, obesity, sexually transmitted infections (STIs), various cancers, prenatal tests, medications that can help prevent HIV, breast cancer, and heart disease, as well as counseling for drug and tobacco use, healthy eating, and other common health concerns. You get free eye exams once per year.

Plan Options:

There are four plan options with deductibles from $1,500 to $7,350. On all plans, the maximum out of pocket is $7,350 (which includes the deductible). On all plans, from day one, you get primary doctor visits for $25, specialist visits for $40, urgent care for $60, generic drugs for $10 and name drugs for between $45 and $85.

They also offer an HSA-compatible plan with a deductible of $5,000 and a maximum out of pocket of $6,550
