HealthMax Plus Defined Benefit Health Insurance
True health insurance - not a discount plan. This plan pays a defined amount for services and procedures.
- Seven doctor visits per year ($100 per visit)
- Diagnostic testing and x-rays ($400 per visit, 5 visits annually)
- Preventative care benefits
- Critical illness benefits ($5000)
- Pays $1000 per day while hospitalized
- Pays $2000 additional benefit 1st day hospitalized
- Pays $1000 per day additional benefit for ICU/CCU (15 days)
- Generic prescription benefits
- Covers 100% of surgery cost (based on Medicare reimbursement)
- Covers you both in and out of the hospital

Plans utilize the MultiPlan PPO network which provides PPO network repricing, and access to over 500,000 providers nationwide. Providers do not have to be in the MultiPlan network, however, for you to be able to use them.
